If you are on long-term medication you will be given a computerised request slip. To order repeats just tick the appropriate medication on your computer slip, place it in the box at reception, fax it or send it to the practice. Your prescription will be ready for collection in 48 working hours.
The practice also offers an email service to request prescriptions. For more information about this service please ask for a leaflet from reception or to request a repeat prescription click on the following link - CCCCG.firstname.lastname@example.org.
Alternatively you may order online via the link at the top of this page. New users are required to register; please ask at reception for your personal registration letter, you will be asked for ID.
Repeat prescription requests will not be accepted over the telephone.
We will send your prescription to you if you provide a self-addressed envelope.
All local pharmacies will collect your prescription for you or have it ready for collection. If you request this service please allow extra time. Some pharmacies provide a free delivery service to those who need it. Ask at the pharmacies.
Hersden Surgery has a dispensary on site for patients who live more than one mile from a chemist. No delivery service is available from our dispensary.
Patients on repeat medication will be asked to see a doctor, nurse practitioner or practice nurse at least once a year to review these regular medications and notification should appear on your repeat slip.
Please ensure that you book an appropriate appointment to avoid unnecessary delays to further prescriptions.
